Post-Secondary Planning & Transition School Improvement

The Regional Transition Network supports schools and cooperating agencies as they work to develop effective transition planning strategies. Through a collaborative effort between State Support Team personnel, guest speakers, and other partners and peers, educators receive coaching to help students prepare for, and achieve successful post-school outcomes in the areas of:

  • competitive integrated employment
  • education/training
  • independent living

Participants will receive important information, news, and updates to improve the fidelity and outcomes of transition planning for students with disabilities, and:

  • explore, identify, and use resources to support writing of compliant transition plans with fidelity
  • develop and maintain partnerships and programming to support transition-age youth with disabilities
  • engage in dialogue and networking opportunities that support cooperative interagency partnerships to promote and support adult behaviors that help to ensure successful post-secondary outcomes for each student
