To fulfill IDEA Part B State Performance Plan Indicator 14 requirements [IDEA Section 1416 (a) (3)] and Ohio law, each school district of residence is required every year to follow-up with each exiting student with a disability within one year of leaving high school to determine student’s engagement in post-school education and employment.
Districts of residence may use the Indicator 14 Exiting Student Data Collection Template to assist in the data collection. The template lists the required data elements and definitions for reporting along with possible questions and prompts that can be asked to obtain the necessary data. The template sheet also includes the valid Education Management Information System (EMIS) reporting options and instructions for each of the required data elements. EMIS reporting is further explained in the FLUP-005 SWD Follow-Up Value Report.
Please note that districts of residence will need to follow-up again with students with disabilities who were also career-technical education concentrators if these students were not reported as engaged during the Career Technical Education and Workforce Development Data Collection.
